If your existing policy is individual, you can migrate the same policy to a family floater policy at the time of renewal. And if your policy is family floater, you can do the same by filling up an endorsement form.
Yes, you can, but only at the time of renewal of your policy. It can be done at the terms of the insurer and please note that the waiting period will apply from scratch for the extra sum insured.

A request needs to be submitted to insurance company & anyone of the below scenario with respective list of proof shall be required:
- Divorce / Separation certificate
- Death Certificate
